What type of Insurances and payments do I take?
I currently am paneled with Aetna, Molina, UHC/ Optum, BCBS, Premera, and community health plan of Washington. I accept debit/credit cards, FSA (Flexible Spending Accounts) and HSA (Health Savings Accounts). Payment is due at time of service. Attention: Please be aware that if you choose to utilize insurance coverage for mental health services, a diagnosis will be necessary. Insurance providers mandate a diagnosis to assess the medical necessity of treatment. It is important to note that mental health diagnoses can potentially remain on your health record indefinitely, as they do not have a statute of limitations. This information is provided to aid in your decision-making process for yourself or your family members. Thank you.
Do you provide Superbills?
I am able to provide superbills for clients. It is important to note that I cannot guarantee reimbursement, or the exact amount of reimbursement, from insurance providers. For those interested in utilizing their out-of-network benefits, I can supply a "superbill" for submission to your insurance or HSA account for potential reimbursement. Process for Superbill: 1. Clients must pay for services in full at the time of appointment. 2. Clients can request a superbill from me. 3. The client submits the superbill to their insurance company for reimbursement based on their individual benefits and coverage plan. Please be aware that a diagnosis must be included on the superbill to demonstrate medical necessity for treatment. Insurance may reimburse anywhere from 20-80%, or even 100%, of the cost depending on the specific out-of-network benefits of the plan. Important Questions to Ask Your Insurance Company: 1. Do I have out-of-network mental health benefits? 2. Is there a deductible? 3. Will out-of-network claims be reimbursed in full, or what percentage will be covered? 4. How long does it typically take to receive reimbursement for a claim? 5. Where should I send the superbill once I receive it from my provider?
What is your cancellation policy?
Please be advised that a 24-hour notice is required for any cancellations. Rescheduling options are available, but may be limited. Late cancellations or no-shows will result in the full session fee being charged. For clients paying with cash, this fee is equivalent to their cash rate. For clients using insurance, your cancelation payment is equal to what your insurance pays. Thank you for your understanding and cooperation.
